Office Policies
- Appointments will be seen first. Walk-ins will been seen after all previously scheduled appointments.
- Your appointment time is reserved specifically for you. While we understand that emergencies and other unexpected situations do arise, we request that you provide your physician’s office with 24-hours’ notice if you are unable to make an appointment and need to reschedule.
- A missed appointment without notifying your physician’s office (no show) or cancellation on the same day of your appointment will result in: $50 late-cancellation fee
- This late cancellation fee will be charged to the patient, not their insurance company.
- This cancellation policy ensures that we can remain consistent with our purpose and promise to provide high-quality, patient-centered care to each of our patients and allows us the opportunity to provide care to another patient.
- You must have your child's Medicaid card at the time of visit.
- No eating or drinking inside of the office.
- Payment is expected when services are rendered unless other arrangements are made in advance.
- We will no longer be able to back date excuses for work/school if we are open for that date of service. If you or your child is sick, please call the office for an appointment or talk with a nurse. If you do not get us on the phone leave a message or send us an email and someone will get back with you.
- We must have all insurance cards on file.
