Mitchell County Pediatrics and Health Center Needs Assessment. We may collect information about your home care needs through the Mitchell County Pediatrics and Health Center Needs Assessment, which may include information such as:

• The person needing care;
• Why the person needs care;
• People available to help locally;
• The amount of services needed; and
• The services you need help with.

We may share the information you provide through the Mitchell County Pediatrics and Health Center Needs Assessment with our franchisees and, if you choose to provide your contact information, a Mitchell County Pediatrics and Health Center provider will contact you regarding your home care needs. By accessing the Mitchell County Pediatrics and Health Center Needs Assessment, you agree to be bound by these Terms. If you are using the Mitchell County Pediatrics and Health Center Needs Assessment, on behalf of another, then you are agreeing to these Terms on behalf of the other and you represent and warrant that you have authority to bind the other to these Terms.

Information Sent to Us by Your Web Browser.

Mitchell County Pediatrics and Health Center recognizes the importance of protecting the privacy of children. By providing your personal information on that Web page you will be consenting to the disclosure of your personal information as described in that Privacy Notice.

As Described in a Click-Through Agreement. 

We may share your personal information as described in any click-through agreement to which you have agreed.

Business Transfers

We may disclose or transfer your personal information to any third party that has acquired the stock or assets of Mitchell County Pediatrics and Health Center or one of our businesses, for example, as the result of a sale, merger, reorganization or liquidation. Governing Law

Mitchell County Pediatrics and Health Center is a United States limited liability company. We control and operate the Site from within the United States. This Policy and our online privacy practices are governed by the State of Georgia, United States of America without regard to its conflicts of law provisions. By submitting your personal information to the Site, you consent to the transfer of your personal information to any country and its use and disclosure in accordance with U.S. federal and Georgia law and with this Privacy Policy.

This English-language version of the Policy is Mitchell County Pediatrics and Health Center official statement of its online privacy practices.
